OF-2682: Prevent modification of shared collection
This modification prevents the org.jivesoftware.openfire.Connection#additionalNamespaces collection being modified upon (first) use. The change intends to reduce the chance of concurrent modification exceptions to occur.
OF-2690: Do not include duplicate namespace declaration with Dialback
Prevents prefix/namespace declaration that already existed on the parent/stream element from being sent out again.
OF-2690: Use the correct Server Dialback namespace
Almost all Server Dialback elements should be qualified by the namespace `jabber:server:dialback`. The namespace being replaced in this commit is only used for a stream feature definition
